Your first question may be “What do you mean...Wellness….for Life?”. Wellness means not only good physical and mental health but financial health as well. We have provided benefit programs that have been designed to help you and your family stay healthy both physically and financially.
Medical Insurance
We offer medical insurance to eligible team members and their dependents. If you have chosen a plan with us, you will have coverage in the event that you, your domestic partner, or your covered dependents need to see a health care provider. You have access to a 24-hour Nurseline, weight management programs with menu planning, disease management programs, discounts to fitness centers, and more! This insurance is paid by you and the Company.
Dental Insurance
We offer dental insurance to eligible team members and their dependents. The insurance is paid by you and the Company.
Pharmacy Program
We offer a pharmacy program to eligible team members and their dependents. A ninety (90) day supply available through the Mail Order Pharmacy. This program is paid by you and the Company.
Retirement Savings Plan (401k)
An account that helps you plan for financial security during your retirement years. Charles Schwab is our current provider. The account can be funded by you on a pre-tax or after-tax basis. You can contribute up to 50% of your pay. You are fully vested in your contributions and any company contributions from your date of hire.
Life Insurance
An insurance plan that provides a financial benefit for your listed beneficiaries in the event of your death. This is provided at one and one half times your annual salary. Accidental Death and Dismemberment is included with this plan.
Supplemental Life Insurance
An insurance plan that will provide additional financial benefit for your beneficiaries in the event of your death. The premium for this insurance is paid by you.
Spouse/Dependent Life Insurance
An insurance plan that will provide additional financial benefit to you in the event of a death of your spouse. The premium for this insurance is paid by you.
Short Term Disability
An income replacement and benefit continuation designed to minimize financial burden of being unavailable to return to work due to illness or injury.
Long Term Disability
An income replacement and benefit continuation designed to minimize financial burden of being unavailable to return to work when your illness or injury lasts for more than six months.
Employee Assistance Program
A confidential referral program that can help you find resources you need to deal with a wide range of personal or family problems.
Health Care Flexible Spending Account
An account that is funded with your pre-tax dollars to cover out-of-pocket health and dental care expenses.
Dependent Care Flexible Spending Account
An account that is funded with your pre-tax dollars to cover expenses for your eligible dependent children, elder care, or disabled dependents. Dependent eligibility is determined by IRS guidelines.
Long Term Care Insurance
Insurance that covers expenses such as a long term care facility or in-home care in the event that you need such assistance for an extended period of time. All team members are covered under the Basic plan of three years up to $36,000.
Bereavement Leave
In the event of the death of an immediate family member you are provided with two working days with pay. One additional day may be allowed if you are required to travel over 150 miles. |
